All content, tools, clinical protocols, screening instruments, navigator workflows, and program frameworks presented on this site are the proprietary intellectual property of Crescent. Aggregate outcome reports and individual screening results generated through the platform belong to the receiving organization or member, respectively. The underlying methodology, clinical design, and program architecture remain ours.
Crescent's tools, screenings, and navigator recommendations are provided for clinical decision support and care coordination purposes only. Nothing on this site constitutes medical advice, a clinical diagnosis, or a treatment prescription. Members should consult their treating providers for all clinical decisions. Employers receive aggregate outcome data for program evaluation, not individual clinical guidance.
Crescent provides this site and its services "as is" without warranties of any kind, express or implied. Crescent shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from the use of or inability to use this site or its services.
These terms are governed by the laws of the Commonwealth of Kentucky. Any disputes arising from the use of this site shall be resolved in the courts of Jefferson County, Kentucky.
